Configuration of the Issues
Navigate to the ‘Configuration’ module. And click on the tab ‘Issues’.
On this page, there are numerous things that can be configured, such as:
Extra issue fields
Issue types
Issue sources
Type of risks
Root cause analysis types
Extra issue fields
Here, you can configure fields that will show in each ‘new issue’. You can think of it as your standard template that will show in each newly created issue.
Issue types
When you create a new issue type, you can select it when creating a new issue. It shows as the first question in the issue. You can configure multiple issue types, and add fields to each individual issue type, that will show up when you are creating a new issue.
For example, we will show the ‘Dangerous situation/near miss’ and the ‘Chemical spill’ issue types. As you can see, each issue type has different questions/fields since you can completely configure them yourself.
Issue sources
Here you can configure ‘Issue sources’. This is a field that will show standard in each newly created issue.
Type of risks
Here, you can configure a standard field for each newly created issue as well. This field is called ‘type of risks’. An example of a risk could be ‘risk for business’.
The ‘type of risk’ field can be found at the bottom of each newly created issue.
Root cause analysis types
In this part, you can configure your RCA’s. For example: 5 Whys, Fishbone or SOAT. Of course you can configure another type of RCA as well. Simply click on the ‘New type’ button, give the RCA a name, and build it in the canvas.
This is an example of a ‘5 Whys’ root cause analysis.
How to report an Issue
To report an issue, you go to the ‘Issues’ module, and click on the ‘New Issue’ button.
Select the ‘type’ of issue:
Fill in all the information regarding the issue, and afterwards click on the ‘Save’ button.
Full overview of all my issues (including resolved)
When you are in the ‘Issues’ module, you see an overview of all the issues with an ‘open’ status. If you would like to see the issues with (all) the statuses, simply click on the ‘status’.
Click on ‘select all’ and after that click on the ‘OK’ button.
Where can I analyze my data?
In the ‘Issues’ module, it is possible to analyze your data. Click on the ‘Open’ button just below the Analysis.
You can add new graphs/tables here. You can also see the total issue cost.
To actually add a new graph, click on the ‘Add’ button.
Add a graph (or table). After that you fill in all the information regarding the graph or table and click on the ‘Save’ button.
As you can see we successfully added a graph in the overview. Now we can carefully analyze this data.
Email notifications for issue owners and followers
As an owner (or follower) of an issue, you will receive a daily summary containing all the logs made in the issue(s). You can expect to receive this overview at least twice per day.
Followers of an issue will not receive immediate email notifications upon issue completion. Instead, these notifications will be sent during the day.
